1. For a visit to occur the: - CRU must have a completed registration form.
- CRU must have current IRB approval and stamped consent form (if a study expires with the IRB before the visit, subjects will not be seen).
- CRU must have a copy of the signed consent form (the consent form may be sent in advance or signed at the visit but no procedures will begin until consent form is signed).
2. Advance Preparation: - Provide the CRU with needed nursing/lab supplies 3-4 days in advance (bring the study kit early so tubes can be checked for expiration, labeled, and prepared for the visit).
- Obtain parking vouchers, if relevant.
3. Subject Reminders; call the subject in advance to:
- Confirm appointment time.
- Review the directions to and the location of the CRU (link to directions).
- Remind subjects of study requirements (e.g. fasting, to bring meds in original bottles).
- Give subject a key word (PI name).
CONTACT CRU IMMEDIATELY IF SUBJECT CANCELS THE VISIT
4. Day of Visit: - Arrive prior to subject’s appointment time to insure everything is ready and to greet the subject.
- Give a copy of the signed consent form to the CRU staff.
- Clean hands upon entering and exiting a room.
- Give the subject a parking voucher.
