Step 4: Scheduling Subjects

Scheduling Subjects at NMH

Complete and submit the following inpatient or outpatient registration forms to schedule subjects at the CRU:

Inpatient Registration Form
Bedded IP Registration Form
Outpatient Registration Form
Off Site Registration Form

1. Before a subject can be scheduled at the CRU, there must be:

  • A NMH research account number.
  • An in-service held for CRU staff with PI and coordinators in attendance.
  • An e-version of final orders sent to the CRU nurse liaison.
  • Final orders signed by the principal investigator.
  • An IRB approval and stamped consent form(s) sent to the Research Subject Advocate.

2.  When scheduling subjects please have 2-3 possible times and dates per subject visit.

3. Tips to Scheduling:

  • Call CRU 1-2 weeks in advance of proposed appointment.
  • Call CRU with 2-3 possible visit times.
  • Confirm tentatively approved time with subject.
  • Send CRU completed registration form to confirm visit.
  • If several visits are planned (e.g. wk 2, wk 4, wk 6, etc) then schedule several visits at one time.
  • No walk-ins allowed!

4. Registration Forms

•    Registration forms must be faxed (312) 926-8450 or dropped by the CRU (Feinberg 10).
        - NMH doesn’t allow the registration forms to be e-mailed
        - The CRU must receive the registration form to confirm an appointment
•    Please fully complete the registration form.

The forms must be entered into the program NMH Cadence. If any fields are missing [e.g. subject date of birth] or if the information is incorrect [e.g. Smythe vs. Smith], a visit cannot be entered. The cadence program does not remember previous entries so:

  • Please completely fill out a form for each visit
  • Make a template for repeat subjects so the form can be easily updated by simply filling in visit date information.
