Electronically submit the required materials to Kelly Carroll:

If you have submitted your protocol via eIRB and listed the CRU/GCRC as a site, you won't need to submit the second set of items.

Required materials to submit electronically to Kelly Carroll:

• CRU submission form for NU studies.
               -See CRU submission instructions for how to get a NIH Common User Name. 
• CRU DSMP form.
• CRU budget worksheet for either:
               -pharma sponsored trial.
               -NIH sponsored trial.
• Lab manual, if applicable.
• Scientific review, if applicable.
• NIH Biosketch, if scientific review has not been conducted.
• Draft Research Orders (Orders Templates and Tips).

If the CRU is not listed as a site in eIRB, the following documents should be submitted:

• New project submission form (unless an eIRB submission).
• Research protocol and/or research plan.
• Investigator’s brochure.
• IRB approval letters, stamped consent(s), and any pending approval letters and PI responses to those letters.
• Any other IRB documentation such as revisions or UPIRSOs.
• Submit a revision to eIRB to add CRU as site.
