- Scientific review, if applicable.
- NIH Biosketch, if scientific review has not been conducted.
- Lab Manual or Manual of Procedures, if applicable.
- Research Protocol and/or Research Plan.
- Investigator’s Brochure, if applicable.
- IRB approval letters and stamped consent(s) as well as any pending approval letters and PI responses to those letters.
- Other IRB documentation such as revisions or UPIRSOs.
- Pharmacy letter of support, if applicable.
